Abstract

Pediatric obesity remains a significant public health concern, particularly in rural areas where access to specialty care is limited. Primary care settings provide an important opportunity for early identification and intervention through family-based lifestyle modification. The purpose of this quality improvement project was to evaluate the feasibility and preliminary impact of implementing the 5-2-1-0 healthy habits framework, developed by researchers at Penn State, in a rural pediatric primary care clinic to promote improved lifestyle behaviors among children with elevated body mass index (BMI).

This project included pediatric patients ages 5–16 years presenting for well-child visits with a BMI at or above the 85th percentile. The intervention consisted of administration of the Healthy Habits Questionnaire, collection of biometric data, and individualized goal setting focused on improving family lifestyle habits. Data were collected at baseline and follow-up visits and analyzed using descriptive statistics and paired comparisons.

A total of 15 participants were part of this project. No statistically significant changes were observed in BMI, weight, or waist circumference during the project implementation period. However, the intervention facilitated structured discussions with healthcare providers and supported individualized goal setting related to diet, physical activity, and other lifestyle behaviors.

Implementation of the 5-2-1-0 framework in a rural pediatric primary care setting is feasible and supports engagement of patients and families in behavior change. While short-term biometric outcomes were not significant, the intervention demonstrated clinical utility in promoting healthy habits. Future efforts should focus on longer follow-up periods, improved follow-up strategies, and integration into routine care to support sustained outcomes
